Our Residential Services provide safe, comfortable, and supportive living environments for adults with special needs. We offer various housing options designed to meet the diverse needs of our residents while promoting independence and personal growth within a caring community setting.

Our homes are thoughtfully designed to feel warm and welcoming, with amenities that support both individual needs and community living. Each residential setting is staffed with compassionate professionals who provide 24-hour support, ensuring residents receive the care and attention they deserve around the clock.

Our staff are trained and complete all required mandatory training to ensure they provide safe, compassionate, and high-quality care while maintaining the well-being and dignity of every individual we support.

Assisted Living


Our Assisted living homes are located at:

  • Queens Creek: 446 Queens Creek Rd, Williamsburg, VA 23185
  • Jolly Pond: 2204 Jolly Pond Rd, Williamsburg, VA 23188

Small residential homes accommodating 6-8 residents, offering a family-like environment with shared common spaces and private bedrooms. Staff members provide round-the-clock support and assistance with daily activities.

Daily Support Services

Our residential program provides comprehensive support services to ensure residents' well-being and personal growth:

  • Personal Assistance: Support with daily living activities such as grooming, medication reminders, and housekeeping.
  • Meal Services: Nutritious meals and snacks prepared daily with accommodations for dietary needs and preferences.
  • Transportation Services: Assistance with transportation to medical appointments, shopping, and community activities.
  • Social and Recreational Activities: Opportunities for residents to participate in group activities, hobbies, and community outings.
  • Independence and Life Skills: Encouragement and support for residents to maintain and strengthen daily living and decision-making skills.
  • Health Monitoring: Staff assist with coordinating healthcare appointments and monitoring residents’ well-being.
  • Community Engagement: Support for residents to remain active in the community and maintain social connections.
  • 24-Hour Supervision: Trained staff available to provide guidance, safety monitoring, and assistance when needed.

Eligibility

Our residential services are designed for Residential and Regular assisted living levels.


Eligibility is determined based on:


  • Must meet Virginia's Legal definition of an ALF. To learn more about what an ALF is scroll at the end of the page.
  • Must choose and qualify for one of two licensure levels:
    • Residential Living Level: Adults needing minimal assistnace. They are capable of maintaining themselves with limited support.
    • Regular Assisted Living Level: Adults needing moderate assistance.
  • Must have active Medicaid.
  • Eligible for auxiliary grants.

Application process includes a comprehensive assessment to determine the most appropriate housing option and support services for each individual. We work closely with families, guardians, social workers, and case workers to ensure a smoothtransition and ongoing success in our programs.

Developmental Disability Waiver Home


Our residential waiver homes are located at:

  • Mother's place: 4989 Hickory Sign Post Rd, Williamsburg, VA 23185
  • Patriots Landing: 7848 Patriots Landing Place, Quinton, VA 23141
  • Seton House: 4801 Huntsman Trail, Quinton, VA 23141

Our residential support services provide 24 hour community based support for adults with developmental disabilities who require assistance to live successfully in our group home setting. Services are delivered in a licensed DBHDS residential facility in accordance with the DD waiver requirements.

Daily Support Services

Our residential program provides comprehensive support services to ensure residents' well-being and personal growth:

  • 24/7 Supervision: Trained staff available around the clock to provide support and ensure safety.
  • Personal Care Assistance: Help with activities of daily living including bathing, dressing, toileting, meal preparation, laundry and medication management.
  • Nutrition Services: Three nutritious meals and snacks prepared daily, with accommodations for dietary restrictions.
  • Transportation: Reliable transportation to medical appointments, activities, and community outings.
  • Recreational Activities: Daily activities and social events designed to promote engagement and enjoyment.
  • Skills Development: Ongoing opportunities to develop life skills.
  • Healthcare Coordination: Collaboration with healthcare providers to manage medical needs.
  • Community Integration: Encouragement and support for participating in community activities.
  • Individualized Support: Tailored care plans to meet each resident's unique needs and preferences.
  • Interdisciplinary Communication: Regular updates and communication with clinicians, families, guardians, and case managers to ensure transparency and collaboration in care.

Eligibility

Our residential services are designed for adults with developmental disabilities who require ongoing support.


Eligibility is determined based on:


  • Have a documented developmental disability.
  • Are enrolled in the Virginia DD Waiver(Medicaid Developmental Disabilities Waiver) or meeet DBHDS (Virginia Department of Behavioral Health & Developmental Services)licensing criteria.
  • Require 24/7 supervision , skill building, and routine support.
  • Benefit from a structured, home-like environment with trained staff.
  • Must have active Medicaid.
  • Must be enrolled with DMAS (Department of Medical Assistance Services).

Application process includes a comprehensive assessment to determine the most appropriate housing option and support services for each individual. We work closely with families, guardians, social workers, and case workers to ensure a smooth transition and ongoing success in our programs.

What is an ALF?

An ALF is a non-medical group residential settting providing or coordinating personal and health care services, 24-hour supervision, and assistance for four or more adults who are aged, infirm, or disabled. ALFs are licensed by the Virginia Department of Social Services and must meet specific regulatory requirements to ensure the safety and well-being of residents.


A resident is eligible for an ALF placemnt when:

  1. They are 18+, and
  2. Their UAI assessment shows either:
    • Minimal Assistance (Residential Living)
    • Moderate Assistance: (Assisted Living)


Choose an ALF if the individual needs:

  1. Moderate ADL (activity of daily living) Support.
  2. Medication Administration.
  3. Structured programs and 24-hour supervised care.
  4. Access to memory care or clinical oversight.

Residents requiring nursing facility level of care are not eligible for ALFs unless they are receiving services through other programs (e.g., hospice) and the ALF can safely meet their needs.
